3 AIMS Immediate: Dissemination of the approach of integrating morphological view into genomic results and reports. Development of methods for NGS reading from minimal amounts of DNA. Development of inexpensive NGS assays requiring minimal primary investment in equipment, to avail small assays to ordinary institutes of pathology.

4 Take home messages to pathologists 1. If you have an H&E slide you have material to work with. 2. Acquisition of high percentage of relevant tumor cells enables to decrease the depth of reading. 3. Digital images are essential for assessment of percent tumor cells. 4. When you send out a block for any molecular test: Get the result Incorporate it in the report Look again at the morphology and correlate. Test both yourself and the result.

Selection of tissue: representation of the tumor in the selected material 3. Quantitative assessment of the amount and percentage of tumor cells (just kidding. When DNA is involved, at least we know what s in the denominator). 4. Surveillance of the molecular process: adequacy of methods. 5. Getting the result: morphology molecular correlation. 6. Integration of result to the report: Control of the match in both directions. Recommendations.

7 Interface #1 Supply of material 1. There are platforms that enable a high yield from very small amounts of DNA (e.g. NebNext Ultra). Can be used if you can provide high percentage of tumor cells. 2. You may recycle the material used in the H&E and immunostains, and extract DNA from your initial slides. To preserve the morphologic information in the slides digital images suffice. 3. Furthermore, when tissue has low tumor cell content and acquisition is challenging, prestaining enables a better richer acquisition than marking areas in consecutive slides. 4. We often encounter the no material left response. Digging into it reveals material. On the other hand: check the requirements. Don t waste the material on platforms that require more than you can provide.

11 Interface 2 # - Tissue selection, representation Tumors are sometimes very variable. When material is abundant, the selection of tumor sites affects the results. Considerations: resemblance to metastases, if available, aggressiveness, inflammation, richness of tumor cells. There are many legitimate considerations, and they should be documented. But in reality, the selection is too often random.

14 Interface #3 assessment of amount and percentage of tumor cells 1. Our ability to assess amount and percentage of tumor cells is not superior to that of non professional. Waste of time. 2. But only we can determine for any given cell nucleus if it is tumor. 3. The efficient way to determine tumor cell amount and percentage is digital, supervised by a pathologist. 4. This approach corresponds and merges with the approach of a robot which can choose a tumor area and dissect tumor cells in it. 5. When an area is selected for extraction of RNA or protein, what is the figure in the denominator? Area? Nuclei? Q: Nobody works like this. How did we overcome so far? A: We worked with sensitive methods, and semiquantitative results: when we are told that 25% of the cells in the sample contain a change, we do not know if 25% of the cells are tumor cells, or if only part of the tumor cells show the change. Pathology enters the digital era, which will solve the simple quantification

16 Interface #4 prediction and morphologic control on molecular process 1. The molecular test is not performed in vacuum, and there are clinical expectations in it. If the results do not make sense or surprise you, inquire. Error can happen both on the molecular and morphologic side, and in the connections. 2. If you get an aggressive report by Oncotype Dx or PAM50 from a tubular carcinoma, activate your suspicion. There is an advantage in documentation of expectations.

17 Interface #5 molecular morphologic correlation. 1. If you are a pathologist who signed a report, and after signing you encounter an IHC stain that you were not aware of: It is clear that you will check this stain, compare the result with the slides you had beforehand, and consider writing an addendum. 2. On the other hand, you send blocks for robust molecular analyses that provide much broader info that an immunostain, and often you don t bother to get the molecular report, read it, integrate it to the pathology report, let alone re-examining the slides 3. We have a lot to learn from looking at slides with known molecular changes. After all, the morphologic changes in cancer derive for the molecular alterations. We may learn how a tissue with distinct molecular fingerprints look, and at the same time supervise the likelihood of potential applications of the results.

18 Interface #5 molecular morphologic correlation. 1. This applies not only to thorough tests such as Foundation One, but also to molecular subtyping of tumors. Experience of parallel morphologic examination with molecular results enables efficient analysis and report of the results. 2. Thou should not assume that the molecular results are always correct.

19 - קורלציה בין הממצא המורפולוגי והמולקולרי. ממשק #5 אם אתה פתולוג שכתב דו"ח, ואחרי החתימה מגיעה אליך צביעה אימונוהיסטוכימית שלא היתה בידיך קודם. ברור לגמרי שתבדוק אותה צביעה, תשווה את הממצא עם התתקינים שראית קודם, ותשקול אם יש צורך בכתיבת דו"ח משלים. לעומת זאת, אתה שולח בלוקים לבדיקות מולקולריות מקיפות, שיש בהן מידע רב פי כמה מכל צביעה אימונוהיסטוכימית. בד"כ אינך טורח לברר את התוצאות, וגם אם אתה מקבל אותן או שאינך מדווח, או שאתה מדווח ללא התבוננות נוספת בתתקינים. יש לנו הרבה מה ללמוד מהסתכלות בתתקינים ששינויים מולקולריים בהם ידועים. גם לעצמנו: ללמוד איך נראית רקמה שיש בה שינויים מולקולריים מסויימים, וגם כבקרה על סבירות הישימות של תוצאות הבדיקה. )מדובר לא רק על בדיקות מקיפות כמו Foundation One אלא גם לבדיקות כגון סוג מולקולרי של סרטן השד ובדיקות ביטוי גנים דומות. ניסיון בהסתכלות מקבילית, מורפולוגית ומולקולרית בתוצאות אלו, מאפשר ניתוח ודיווח יעילים של התוצאות. אין להניח שהתוצאות המולקולריות תמיד נכונות

20 - אינטגרציה של המידע המולקולרי לדוח הפתולוגי. ממשק #6 המורפולגיה משקפת שינויים מולקולריים. ככל שנדע יותר פרטים מולקולריים בהסתכלות המורפולוגית שאנו רגילים בה, נמצא יותר ויותר קורלטים. לחלק מאלו נוכל לקרוא בשם, כמו הקשר בין signet ring cell appearance של אדנוקרצינומה של הריאה לבין טרנסלוקציה ב- ALK. בהרבה מצבים אחרים לומדים להכיר את המופעים גם ללא יכולת לתת להם שם ברור. תוך זמן לא ארוך אני צופה שתתפתח גם פתולוגיה דיגיטלית עם יכולות למידה עמוקה, שתשפוך אורות נוספים על פרדיקטביליות של ממצאים מולקולריים. ייתכן מאד, שקורלטים מורפולוגיים יהפכו יותר יעילים מהבדיקות המולקולריות עצמן בפרדיקציה של תגובה לטיפולים מסויימים. אותם קורלטים מורפולוגיים משקפים לעיתים מסלולים ביוכימיים פיסיולוגיים, ולא רק מוטציות בודדות.

21 Interface #6 Integration of molecular info into the report. Morphology reflects molecular changes. The more we know molecular results of slides we look at, the more correlates we will find. Some of these correlates have names, such as signet ring cell appearance in ALK-translocation derived adenocarcinoma of lung. In most cases, we will learn to identify specific fingerprints without definng or naming the changes. Shortly, it is expected that digital pathology with deep learning modalities will shed new lights on the predictability of molecular findings, on the background of morphologic changes. Morphological correlates may in some cases be more efficient than the molecular tests in the prediction of response to drugs: the morphology may broadly reflect target biochemical pathways, and not only specific mutations.

22 ומשהו על זמינות, שכיחות ומחיר: היום בדיקות מולקולריות מקיפות נעשות בעיקר לחולים גרורתיים, בשלב מתקדם של המחלה, ולחולים לקראת טיפולים חדשניים ויקרים. ככל שהשיטות תוזלנה, הן יכולות לשמש אותנו ככלי באבחנות רוטיניות. כדי לקדם תהליכים כאלו, יש צורך בשיטות שתדרושנה מכשור זול בהרבה, ושתאפשרנה להיות cost effective גם בעומס בדיקות של מכון פתולוגי בינוני או גדול, ולא רק במרכזים גנומיים. To date, robust molecular analyses are performed mainly to metastatic, advanced cancers, in patients seeking innovative and expensive treatments. The cheaper the methods will become, the more useful they may be in routine pathology.
